Organ Donor Registries

There are three ways to give your consent

Register with the Régie de l’assurance maladie du Québec (RAMQ) organ donor registry, the Registre des consentements au don d’organes et de tissus. To ensure that your decision is honoured and to officialize your consent, register with the Régie de l’assurance maladie du Québec (RAMQ) organ donor registry, the Registre des consentements au don d’organes et de tissus. Simply sign the organ and tissue donation consent form that will be mailed to you along with your RAMQ card renewal. You may also request one by calling RAMQ at 1-800-561-9749. All information provided to RAMQ is treated confidentially. When the time comes, Transplant Québec and Héma-Québec will be able to check quickly, at the request of a physician or other authorized person, whether your consent has been registered. A person who has given his or her consent is entitled to revoke that consent at any time. The form for revoking consent to organ and tissue donation is available from RAMQ.
2.Sign the sticker and affix it to the back of your health insurance card. You may give your consent by signing the sticker and affixing it to the back of your health insurance card. This sticker is sent to you along with your new health insurance card at renewal time. The stickers can also be found at a number of healthcare institutions, on Signez-don! pamphlet published by the MSSS, and from the various organizations that promote organ and tissue donation.

Register with the organ donor registry managed by the Chambre des notaires du Québec. will or a mandate in case of incapacity, take a few moments to consider donating organs and tissues. If you consent to organ and tissue donation after discussing it with your notary on your own accord, he or she can note it in your will or mandate in case of incapacity. This consent will then be added to a specially created registry established by the Chambre des notaires du Québec. At the right time, authorized personnel will be able to verify whether a potential donor has consented to organ and tissue donation; keep in mind that the privacy of your consent information is always protected. Regardless of how you give your consent, you should always speak to your loved ones about your decision.
